Family Violence 2016 Prevention Initiative Survey Documents
The Division on Domestic Violence conducts assessments of its Family Violence Prevention Initiative (FVPI) funded programs. These include ongoing monitoring of service evaluations that are completed by program clients. The Evaluation of Services Survey (ESS) is designed to capture client satisfaction and outcomes. Delegate agencies should have client participants complete this form, keep the original and return a copy to:
