Ombudsman Program
The City of Chicago has a dedicated team that represents individuals who live in long-term and residential care facilities (including skilled nursing, assisted living, supportive living, shelter care, and other specialized facilities). If you have concerns about the care or services received by an older adult or an individual with disabilities aged 18-59 who lives in a long-term or residential care setting, you can contact an Ombudsman for assistance:
