DESCRIPTION: Department of Family and Support Services (DFSS),
[Caregiver Respite Services RFP]
The Chicago Department of Family and Support Services (DFSS) seeks organizations who have experience managing a comprehensive, citywide respite program including outreach, assessments, creating person-centered plans of respite care, and coordinating the most appropriate in-home and facility-based respite care for caregivers of any age who are providing support to adults 60 years of age or older or to an individual under 60 with Alzheimer’s disease or a related dementia. Respondents may include, but are not limited to, for-profit or non-profit organizations, universities, hospitals, health care providers, faith-based organizations, and / or organizations that support older adults and / or their caregivers. The selected Respondent is expected to deliver 25,000 hours of respite care and 350-400 assessments per fiscal year for 350-400 participants based upon the current rate of services, and target outcomes will include 80% of caregivers self-reported reduced stress and 80% of caregivers report they are able to continue to provide care for the care recipient as a result of the intervention.
The term of contract(s) executed under this RFP will be from October 1, 2024 –September 30, 2027. Based on need, availability of funds, and contractor performance, DFSS may extend this term for up to one year with the extension not to exceed one year. Additional funding may be dependent upon the selected Respondent’s performance and availability of funding. DFSS anticipates funding one delegate agency to be awarded $550,00 annually, depending on the size of the proposed program.
