DESCRIPTION:  Department of Family and Support Services (DFSS),

Senior services: Aging well navigators and gap-filling services

 

The Aging Well Navigators and Gap-filling RFP seeks proposals from qualified community- based organizations including but not limited to social service providers, university institutions, hospitals, community health organizations, not-for-profit, for-profit institutions and other providers who have experience providing benefits and social services resources, Gap-filling services and other relevant supports to adults 60 years of age or older.  

 

This program seeks to help older adults identify benefits and social service resources that support well-being and quality of life assisting them to live independently in their homes and communities for as long as it is safely possible to do so. The program is implemented by sending Aging Well Navigators into senior buildings across the city to serve as on-site resource specialists that will help older adults become aware of and apply for benefits and senior services resources available to them.

 

The selected Respondent will be responsible for program operations and management including, outreach, recruitment, assessment, program implementation, staff retention and data collection on the impact of the program on the older adults served. Aging Well Navigator provider agencies will operate within 4 regions of the city serving at least 2,400 seniors per region. They will receive a list of buildings for program consideration as generated through the ADPI program. DFSS may also receive referrals and/recommendations to add onto the list from Alderman and concerned friends, families, community residents and other stake holders.

 

DFSS anticipates up to four awards for $265,500 per region and inclusive of costs for two full time case management navigators, programming supplies (laptop and Wi-Fi), travel reimbursement, language translation support and outreach materials, background checks (including fingerprinting and APS registry), and administrative costs, and $85,500 for Gap-filling funding within each service Region.

 

Successful applicants will be scored based on multiple criteria, including anticipated numbers served, as well as the plan for regional service delivery. Respondents seeking funding for all four regions are required to apply for each region separately. Respondents should include best practices and evidence-based supports, an outreach plan, a training and implementation plan for supervision and support. Models may include paid, volunteer and/or in-kind professional and lay persons. Plans should also include a process for conducting assessments of older adults and an impact and evaluation plan.

This RFP will be open for response starting at 9am on Tuesday, November 21, 2023 and will close at Noon CST on Tuesday December 5, 2023. The term of contract(s) executed under this RFP will be from January  1, 2024 – December 31, 2024.
