Direct Pay Program
As required under the Family Medical Leave Act of 1993 and under other circumstances as outlined in the City’s Healthcare Plan, when an employee is not actively at work (on an approved leave of absence), employees may continue medical, dental, and vision coverage, through the City’s Direct Pay Program. PHSA/COBRA
When coverage under the City’s Healthcare Plan ends, medical, dental, and vision benefits may continue, depending on the circumstance, at the Participant’s own expense for a temporary period. To be eligible, first a qualifying event causing the loss of coverage must take place, and then the qualified beneficiary must enroll in and pay for these benefits, by the deadlines. Alternative Coverage Program
If an employee applied for coverage for themselves or an otherwise eligible dependent and was denied coverage because of failure to meet the enrollment or documentation deadlines of the Plan, the employee may be eligible to enroll in Alternative Coverage. Alternative Coverage is identical to regular coverage except that the employee will be required to pay a higher premium. All relevant Plan terms will apply. See Plan document for more information.
