Welcome!

Please join the newly assembled Syndemic Infectious Disease Bureau (SIDB) at CDPH, Tuesday, May 2nd from 8:30-4, for a free one-day advanced continuing education program on the evolution of screening and treating Syndemic infections in Chicago. Federal, state, and local leaders in public health and Syndemics prevention will discuss trending topics of local and national importance, including Syndemic infectious disease epidemiology, application of research findings to programs and practice, and strategic public health interventions among the Syndemic infections. The SID bureau will also host onsite personal technical assistance to ensure providers have all the tools they need to stay on trend and stay connected.

Who do I contact for questions about the venue?

You will find information about Malcolm X, including information about getting to the venue, taking public transit, and free validated parking here. If you have additional questions about the event venue or space, please reach out to a member of our Conference Committee at ashley.becht@cityofchicago.org.

Is parking at the venue free?

Free parking is available inside of the Malcom X garage, situated right next to the conference center. On arrival CDPH volunteers will be on site to greet you, check you in and validate your parking, if you are parked in this garage. There is street and nearby parking that is paid as well, but those fees are not covered by Malcom X nor CDPH. How will I register for and obtain CME/CEU credits?

Registration for any applicable CMEs is collected at the time of registration. The St. Louis STI/HIV Prevention Training Center is providing CME credit through the MSMA. Credits for this training will be provided by them upon completion of the post-conference assessment; a link for which will be sent via email from CDPH May 5th, 2023.

*Note, for providers seeking credits who do not hold an MD, the St Louis PTC will provide a certificate of attendance and CDPH will assist in validating your participation in any way we can.

Where can my organization find more information about SID Condition reporting?

The CDPH Office of Surveillance is responsible for reporting morbidity to the Illinois Department of Public Health [IDPH] and the Centers for Disease Control and Prevention [CDC]. As a provider, the expectation is to report accurate and timely information so as to ensure adequate tracking and identification of disease prevalence in high morbidity communities. This collaborative effort between the provider and the Office of Surveillance makes it possible to develop and provide recommendations that maximize disease control interventions.

What is the best way to report morbidity to the health department?

The CDPH Office of Surveillance is responsible for reporting morbidity to the Illinois Department of Public Health [IDPH] and the Centers for Disease Control and Prevention [CDC]. Reporting to the Office of Surveillance occurs electronically through the Illinois’ National Electronic Disease Surveillance System (I-NEDSS) or via the Chicago Health Information Management System (CHIMS). In 2021, the Chicago Department of Public Health (CDPH) Syndemic Infectious Disease Bureau, Office of Surveillance started accepting confidential syphilis, congenital syphilis, and HIV/AIDS case report forms electronically via the Chicago Health Information Management System (CHIMS) Provider Portal. In 2023, electronic reports for viral hepatitis B & C can be submitted via CHIMS. Healthcare professionals (and designees) will need to apply and be approved for a Provider Portal account via the CHIMS Provider Portal Account Application System before morbidity reports can be submitted via CHIMS. The Syndemic Infectious Disease Bureau has established a strong, working relationship with national network of HIV/STI Prevention Training Centers. This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Missouri State Medical Association (MSMA) through the joint provider ship of St. Lous STD/HIV Prevention Training Center and the Chicago Department of Public Health. The St. Louis STD/HIV Prevention Training Center is accredited by the MSMA to provide continuing medical education for physicians.
