The CDPH Office of Surveillance is responsible for reporting morbidity to the Illinois Department of Public Health [IDPH] and the Centers for Disease Control and Prevention [CDC]. As a provider, the expectation is to report accurate and timely information so as to ensure adequate tracking and identification of disease prevalence in high morbidity communities. This collaborative effort between the provider and the Office of Surveillance makes it possible to develop and provide recommendations that maximize disease control interventions.

Reporting to the Office of Surveillance occurs electronically through the Illinois' National Electronic Disease Surveillance System (I-NEDSS) or via the Chicago Health Information Management System (CHIMS). In 2021, the Chicago Department of Public Health (CDPH) Syndemic Infectious Disease Bureau, Office of Surveillance started accepting confidential syphilis, congenital syphilis, and HIV/AIDS case report forms electronically via the Chicago Health Information Management System (CHIMS) Provider Portal.
