For those employees who are an approved leave of absence for longer than 30 days, please contact the Chicago Benefits Service Center at 1-877-299-5111 or go online at www.cityofchicagobenefits.org within 30 days following the employee’s return to work in order to reinstate medical benefits. All family members previously covered will be reinstated also. Separate enrollment forms and certified documents are not required again to reinstate coverage for a spouse, domestic partner and/or dependents.
