Contact a City Forester at the Bureau of Forestry for all Permits.
The Bureau of Forestry issues permits for any work involving trees. This requires the following information for permit issuance.
A Certificate of Liability Insurance is required from the Landscaping Company or Sub-Contractor performing the work within the public way. An endorsement of the certificate must name the City of Chicago as additionally insured.
The description on the certificate should read as: *Minimum $1,000,000.00 General Liability for each occurrence
This Certificate Endorses the City of Chicago as Additional Insured
The Certificate Holder Shall Be: The Department of Streets and Sanitation Bureau of Forestry - Permit Division 2352 South Ashland Avenue, Chicago, Illinois 60608
Forestry will accept a facsimile of your Certificate of Liability Insurance for initial registration only. Your faxed copy will only be good for seven (7) days. Request an original Certificate of Liability Insurance be sent to Forestry as soon as possible. Forestry must receive an original Certificate of Liability Insurance within seven (7) working days. We will keep your Certificate of Liability Insurance on file until the expiration date stated on the original document.
Complete the Parkway Tree Work Permit Request and fax a copy to Forestry at 312.743.8044. Please indicate if the work to be performed is on Public Property or Private Property. Please allow us at least 24-hours notice per request, and 10-working days for removal requests.

A copy of the Permit must remain in the vehicle at all times.
