Healthy Chicago 2025 Strategic Report

DPH has released the Healthy Chicago 2025 Strategic Plan report, which aims to reduce the racial life expectancy gap. On average the life expectancy of Black Chicagoans in 2022 was 11.4 years less than their non-Black counterparts. The largest drivers of this gap include chronic disease, homicide, and opioid overdose. The Healthy Chicago 2025 Strategic Plan report features six programmatic action plans aimed at addressing the key contributors to the life expectancy gap Chronic disease:

  • Violence prevention
  • Substance use
  • Infectious disease
  • Infant and maternal health
  • Mental health
  • Partnerships

Communities that have faced significant health disparities, including East and West Garfield Park, North Lawndale, Englewood, and West Englewood, are specifically targeted for these interventions. Priority communities were chosen due to lowest life expectancies and greatest declines in life expectancy in recent years.

By confronting the primary drivers of the life expectancy gap among the people and places that have experienced the most disinvestment and health decline, CDPH aims to reverse the historic trend of divergent health outcomes and promote the wellbeing and prosperity of Chicago and all its residents.

CDPH invites residents to review the Healthy Chicago 2025 Strategic Plan and provide feedback from December 16, 2024, to January 6, 2025. Comments can be submitted via email to healthychicago@cityofchicago.org. CDPH staff will review all submissions and address questions.
