Motion to Set Aside the Default

General Order 20-3 from the Chief Administrative Law Judge provides that Motions To Set Aside defaults can be submitted via email.

Please complete the MTSA form and choose one of the three following options:

     1.) email it to AH-MTSA@cityofchicago.org

     2.) fax to 312.742.8222,

     3.) file in person at 400 West Superior.

Select ONLY ONE of the above options.  For email or fax requests, please allow at least three business days for the MTSA to be processed. Resubmitting another request will delay the process.  Therefore, do not email,  fax or mail another copy of your MTSA.

DOAH will then schedule a hearing date on which this motion will be heard. A notification will be sent to your email. Please include a daytime phone number on the motion.
